End-of-Life Care Sub-Committee Meeting

Furthering the work of the Walker County Health Action Partnership —  Meeting to engage chaplains; social workers; hospice, home-health and long-term care admissions nurses, case managers, pastors, physicians and others who are in conversation with people about end-of-life/advanced-directive-care decisions. The End-of-Life Care Sub-Committee is a part of the Coordinated Care Priority group.
